Incident Narrative

An employee was balancing 15 4x8, 3/4-inch-thick sheets of plywood while another employee attempted to remove one sheet from the stack. The sheets of plywood then slipped and fell onto the employee's left foot. He sustained fractures to four toes, the top of his foot, and possibly his left ankle.
