Incident Narrative

An employee was working from an extension ladder to hang drapes on second-floor level windows when he transferred from the ladder to a small plywood ledge. The ledge gave way and he fell backwards, landing on top of a folding table. The table then broke and the employee landed on the floor. His tape measurer was on his belt and it impacted his lower spine. He was hospitalized with a fractured coccyx.
