Incident Narrative

An employee was adjusting the chuck jaws of a vertical mill so the material would fit and was placing a 24" flange into the chuck of the mill. While she was removing the sling strap, the piece slipped and caught her right-hand first fingertip at the base of the nailbed between the material and chuck jaw, resulting in an amputation of her fingertip.
