Incident Narrative

An employee was loading canopy deck pans onto a truck and placing cardboard spacers between the canopies to protect the surface. In order to place the spacers, he was standing on the canopy deck pans. His right foot became caught on the edge of a deck pan, causing him to lose his balance. He then fell approximately 10 feet onto the gravel parking/loading area, fracturing his left arm, breaking several ribs, and bruising his left hip.
