Incident Narrative

On 10/11/2018, an employee was observing and recording information about a conveyor drive motor while standing on a step ladder. The conveyor line was located approximately 9 feet above the floor. The sleeve of his smock became caught in the line, causing him to lose his balance and fall from the ladder. He was hospitalized with a laceration and fracture to the left arm.
