Incident Narrative

On October 2, 2018, an employee was doing quality control checks on shotcrete application, using a welding rod as a probe to measure the thickness of the shotcrete. Shotcrete material separated from the face of the excavation and struck the employee on the head, neck, and face. The employee suffered fractures to the face, neck, and leg.
