Incident Narrative

On 9/20/2018, an employee was using a spatula to break up clumps of sugar in a commercial mixer when the mixing blades grabbed the spatula, pulling the employee's hand into the blades and resulting in an amputation of the left index fingertip. The mixer was running at the time of the incident.
