Incident Narrative

An employee was un-banding a coil on the forks of a forklift. When the banding was cut, the coil fell forward and the employee was caught between the mast of the forklift and the coil. The employee was hospitalized with a broken clavicle, scapula, and finger, as well as broken ribs.
