Incident Narrative
A forklift operator had walked away from a forklift when an employee noticed the forks were covered with gravel. The employee was removing the gravel by hand when the forklift operator returned and began operating the forklift, unaware that the employee was below. The forks then crushed the employee's right index finger, resulting in a medical amputation of the fingertip.
