Incident Narrative

An employee was attempting to replace a pressure gauge on a hot gas ammonia line. As the employee loosened the gauge for removal, a quarter-inch pipe and valve broke, releasing ammonia vapor above the processing floor. The employee inhaled the ammonia vapors and required hospitalization.
