Incident Narrative

Two employees were working from an aerial lift to install insulation within the interior walls and roof of an aviation hangar. The lift operator drove it into an existing two-foot deep excavation causing the lift to tip over. Both employees fell out of the lift basket onto the concrete hangar floor. One employee was hospitalized for multiple leg fractures. Both employees were wearing a personal fall arrest system at the time of the incident.
