Incident Narrative
At approximately 5:40p.m., an employee (positioned on the south end) was moving a laydown cart to the home position (approximately 20 feet long) with a co-worker (positioned on the north end). When the employee reached the end of the track (home position), the cart closed in on itself (accordion style cart) and pinched his right thumb between the cart frame and extension, resulting in an amputation of the right thumb tip and distal phalange.
