Incident Narrative

An employee was working on a raised platform, tightening a pipe with a pipe wrench. As he was pushing the wrench forward, the wrench slipped, which carried him forward and caused him to fall approximately 3 feet. He fractured his right femur. The platform did not have guardrails at the time of the incident.
