Incident Narrative

An employee was using a load arm to transfer a test joint from a transfer line to storage when a bolt came loose on the load arm, causing the test joint to fall out of the load arm and onto the rollers and the table. The employee then fell backward to the ground and hit his head, losing consciousness.
