Incident Narrative

On August 24, 2018, at around 1:57 p.m., an employee was cleaning a line using a central vacuum system. She went to the discharge side of the system (where scrap barrels get changed out manually) and was reaching through the collar when she made contact with the rotary valve. The moving valve amputated her right index and middle fingertips.
