Incident Narrative

An employee was on his hands and knees, doing maintenance work on a roof. He stood up, became dizzy and then fell off the roof, landing 12-15 feet below. He suffered broken ribs, a broken pelvis, a head contusion, and a cut to his right little finger. At the time of the incident, the employee's harness that was not tied off.
