At approximately 11:15 a.m., on August 17, 2018, an employee was attempting to clear a powder buildup that was stuck in a dryer's rotary valve when the employee's right-hand fingers contacted the rotary valve, amputating the ring, middle, and pinky fingers. The rotary valve was not turned off and/or locked out at the time of the incident.
