Incident Narrative

On August 15, 2018, at approximately 10:30 a.m., an employee was attempting to clear a jam in the feeding section on a box former machine when the employee's gloved hand was caught by the machine's shuttle carrier, causing injuries that required surgical amputation of the middle finger on the employee's left hand. The machine was guarded at the time of the incident.
