Incident Narrative
An employee was watching a second employee assess a noise from a cut-off saw. The first employee grabbed the blade because he thought it looked loose. At the same time, the second employee "bumped" the saw, which caused the blade to grab the first employees right index finger and amputate the tip. The saw was not de-energized at the time of the incident.
