Incident Narrative

An employee was using a forklift to load a semi-truck trailer. The employee set the forklift brake, got off of the forklift, and walked to the front of the forks. The forklift moved forward and pinched the employee's left calf between the double forks of a slip sheet attachment. The employee suffered a left calf laceration and was hospitalized.
