Incident Narrative

An employee was trying to install a new industrial burn-out oven. He climbed on top of a section of the oven and was using a pry bar to pry another section of the oven out when the pry bar slipped and he fell approximately 12.5 feet to the ground. He broke his nose and both wrists and suffered a cut above the eye.
