Incident Narrative

An employee was dumping a 50-pound bag of solid particulate into a dump station. The employee's left hand contacted a butterfly valve in the bottom of the station. The valve was activated, and the employee's middle and ring fingers were amputated below the fingernail. The valve was not guarded with a screen.
