Incident Narrative

An employee had placed one foot on a pallet, about 3 inches above the floor, to retrieve banded cardboard. When the employee turned and stepped down, they twisted and broke their ankle. The employee was hospitalized, requiring pins in their ankle. Low-cut shoes, not boots, were being worn at the time of the incident.
