Incident Narrative

An employee was checking the vacuum on a machine when he noticed that the water level was getting high in the slurry. He discovered that pellets were clumping together; when he tried to stir the water, he received an electrical shock caused by a wire that had shorted out. He suffered head and neck pain and was hospitalized.
