Incident Narrative
An employee was installing an air handler on a building's second floor using an all-terrain forklift. The equipment had been placed on a 12-by-12 skate and moved to a placement pad. It shifted and struck the employee, causing fractures to the wrist, elbow, and leg, and a laceration to the forehead.
