Incident Narrative
An employee was operating a pump supplying water for a fracturing operation (oil/gas) when the pump's discharge hose disconnected, and the hose clamp securing the hose to the flange failed. The hose then struck the employee's right shoulder, upper right back, and pelvis. The employee suffered two fractures to the pelvis, contusions, and abrasions, requiring hospitalization.
