On June 24, 2018, an employee was working in the warehouse to prepare the upcoming week's jobs. He used an extension ladder to try to retrieve a dolly from a box stored on a mezzanine. The ladder slipped out and the employee fell to the floor, breaking his hip and foot. His injuries required surgery and hospitalization.
