Incident Narrative
Employees 1 and 2 were using a winch to load equipment onto a trailer. As employee 2 went to loosen the winch, employee 1's hand went between the equipment and the winch strap. Employee 2 accidentally tightened the winch instead of loosening it, trapping employee 1's left hand and resulting in a left index fingertip amputation.
