Incident Narrative

An employee was changing a die in a machine. After locking/tagging out the machine, the employee removed a pin securing the die in place. While the employee's hand was between the upper and lower dies, the upper metal die fell due to gravity and crushed the employee's hand between the two dies. The employee's hand suffered bruising.
