Incident Narrative
An employee was connecting a new transformer while in a bucket truck. The pole was a three phase tangent with a single phase tap. The employee's arm contacted the bottom of the cut out fuse serving the single phase line. The line fuse blew on contact, and the employee suffered burns to the arms and back that required hospitalization.
