Incident Narrative

An employee was performing a headboard change out on a mold machine. The employee stepped back onto a limit switch causing the hydraulic pusher to traverse and catch the employee's left foot between the 900 psi hydraulic pusher and a fixed portion of the roller conveyor. The employee's left foot was fractured. The conveyor with hydraulic pusher was not locked out/tagged out at the time.
