Incident Narrative

A RackMasters employee and a temporary employee were in a scissor lift at an elevation of approximately 20 feet, using a hand-held tool to unscrew angle joints and dismantle a rack. They were removing the final piece of the rack, which was against the wall of the facility (almost flush with the wall), when the rack became loose, fell, and hit the scissor lift. The scissor lift then fell over and the two employees jumped when it was about 2 feet from the ground. The employees suffered a spinal fracture and fractured arm.
