Incident Narrative
Two employees were performing a routine inspection of the neutron generator. One of the two workers heard a hissing sound and they detected a leak that could be corrected by tightening a flange bolt. When one of the employee's set the wrench on the flange bolt, the clamp failed and the internal pressure propelled the dome (globe) away from the base and the dome struck the other employee (without the wrench) in the chest. The injured employee sustained one broken rib and a punctured spleen.
