Incident Narrative
An employee was attempting to wind a hose onto a pneumatically actuated drum. While tying a knot onto the hose with string, the pneumatic drum actuated and applied tension to the string. The employee's finger was caught in the string as it tightened, amputating the flesh off his index fingertip.
