An employee was attempting to remove a spare tire carrier from underneath a minivan that was elevated by four wheel stands when the minivan shifted off the front loader forks and struck the employee's face and shoulder. The employee suffered a fractured eye socket and left-side collar bone and lacerations to the head and face.
