Incident Narrative

A mechanic was opening the slide gate of a plastic extruder machine when the heat zone element overheated the plastic up to 900 degrees instead of 400 degrees. The hot plastic sprayed out and onto a different employee who was approximately 10 feet away. The injured employee suffered burns to the arms and face.
