Incident Narrative

An employee was working from the extended boom/basket of a bucket truck. The outriggers of the bucket truck were not set on solid ground, resulting in the outrigger sinking into the ground as the boom was further extended. The truck became unstable and collapsed with the employee inside the basket. He rode the bucket to the ground and fractured his back and ribs.
