On May 17, 2018, an employee and a coworker were preparing to move steel plates. The coworker had the steel plates lifted with a pry bar while the employee's hand was placing a clamp under them. The coworker removed the pry bar, which caused the steel plates to fall onto the employee's left middle finger and resulted in an amputation.
