On 5/14/2018, a temporary employee was walking through a warehouse when his phone was knocked out of his hand. As he reached to pick up his phone, a powered industrial vehicle backed over his left foot, breaking it and requiring surgery. Steel toe shoes were worn at the time of the incident.
