Incident Narrative
An employee was operating a 1,250-ton press brake to bend parts. As a part was being bent by the press dies, it swung up toward the face of the ram. The employee was still holding the part, and his right middle and ring fingertips were then pinched and amputated between the part and the front of the machine.
