Incident Narrative
On 05/14/2018, at approximately 2:45 PM, an employee was working from a 12-foot fiberglass A-frame step ladder. The employee was using a drill to install conduit for a junction box when he fell 8-9 feet from the ladder onto the concrete floor below, fracturing his right/left wrists and lacerating his face above the left eye. He was hospitalized for surgery and stitches. Proper PPE was worn at the time.
