Incident Narrative
An employee was pulling a pallet down from a rack with a reach truck. He reached up to adjust a light with his left hand while his right hand was on the joy stick. He unintentionally pulled back on the joy stick and the truck went backward, catching his left hand between the rack and beam behind him. His left pinky finger was amputated.
