Incident Narrative
An employee used a forklift to stack pallets on a stack and was backing up when he thought he was going to hit a desk and drove forward instead. The floor had condensate and the forklift skidded into his foot, crushing it between the forklift and an angle iron that was installed on the floor. He suffered a fractured talus bone, requiring hospitalization and surgery.
