Incident Narrative
A temporary employee was taking glue and paper from the cutter end of a pleater machine. Another employee was at the pleater belt end of the machine. The employee at the pleater belt end rolled the belt and her right index finger went under the knife, amputating her fingertip. The machine was unguarded at the time.
