Incident Narrative

An employee was conducting preventative maintenance on a fan bearing. While circulating the grease, the employee grasped the fan belt and manually manipulated the fan. His right index and ring fingers were then caught between the fan belt and the pulley. He sustained a partial amputation to the right index finger and a fracture to the right ring finger.
