Incident Narrative
An employee had been cutting through a section of a conveyor that was being removed. He was using a reciprocating saw, handheld grinder, and ladder. He removed one foot from the ladder and placed it on a metal stationary ladder that was inches away. He had one foot on each ladder when the metal ladder shifted, causing him to fall. His left wrist was fractured in two places and he sprained his back.
