Incident Narrative

An employee was performing routine maintenance on a process vessel. While the employee was lowering the vessel head plate into position over the vessel, a member of the lifting mechanism unexpectedly shifted, causing the employee's right ring finger to become pinched between the mechanism and the head plate. The employee's finger was partially amputated.
