Incident Narrative

On 4/7/2018, at 2:30 p.m., an employee was stripping concrete dismantling form-work. He removed a U-bolt and was climbing down a shoring frame when he slipped and fell about 10 feet to the ground, fracturing his left wrist and requiring hospitalization. He was wearing harness and lanyard and was anchored to the shoring frame at the time of the incident.
