Incident Narrative
An employee was using a "tugger" to help another employee move two-tier racks with a forklift. The forklift knocked over the top tier of racks. The employee using the tugger fell while getting out of the way of the falling racks or the racks struck the employee and he fell. The employee sustained a fractured jaw.
