Incident Narrative

An employee was hand feeding strips of steel into a full-revolution press and punching out multiple parts from a single strip. He was trying to remove the raw material skeleton blank from the die when the press cycled, causing the die to close on his hand. The die came to a stop at the bottom of the stroke, trapping his hand in the die and severing his left middle fingertip.
